Globulins are every protein in your blood except albumin — the antibodies that defend you, the carriers that move iron and hormones, and the proteins that rise with inflammation. Most people meet the word on a comprehensive metabolic panel, where globulin appears next to total protein, albumin, and the albumin-to-globulin (A/G) ratio. A globulin that sits slightly outside the range is common and rarely serious on its own. What are globulins?

Your serum holds roughly 6.0–8.3 g/dL of protein. Albumin accounts for a little more than half; everything else is globulin.1 So the word names a family, not a single protein — hundreds of different molecules grouped by the lab technique that separates them.

That technique is electrophoresis: proteins migrate through an electric field and gather into bands by charge and size, and the lab measures how much protein sits in each band. Modern analyzers use capillary electrophoresis, which is more reproducible than the older agarose gel.8

Your globulin number is usually subtracted, not measured. On a metabolic panel, the lab measures total protein and albumin, then reports globulin as the difference. A globulin that looks slightly off can therefore simply reflect a shift in albumin.2

The albumin-to-globulin (A/G) ratio

The A/G ratio divides albumin by globulin. Because albumin is normally the more abundant of the two, the ratio is greater than 1 — in practice about 1.0 to 2.0, depending on the lab.1

A low A/G ratio is the common finding, and it has two very different explanations that the ratio alone cannot separate: albumin went down (liver disease, malnutrition, nephrotic syndrome, prolonged inflammation) or globulins went up (chronic infection, autoimmune disease, a monoclonal gammopathy). A high A/G ratio points instead to low globulins. Research keeps linking a low A/G ratio to less favorable outcomes across several diseases, but the honest reading is narrow: it is a broad reflection of nutritional and inflammatory status, not a diagnostic test, and emphatically not a cancer screen.3

Why globulins are measured

The test is rarely ordered on its own. It comes up when total protein is abnormal and the question is which fraction; when inflammation persists with a high CRP or ESR; when recurrent infections raise concern about low antibodies; in follow-up of liver disease, since the liver makes most blood proteins; and when a monoclonal gammopathy is suspected — in which case serum protein electrophoresis is the key test.4

Interpreting your results

High globulin

A high total globulin most often reflects a chronic immune stimulus: a long-standing infection, an inflammatory or autoimmune disease, or chronic liver disease — in cirrhosis, the beta and gamma bands merge into a characteristic "beta-gamma bridge." Dehydration can nudge every protein up at once. When the elevation persists without an obvious cause, the next step is usually SPEP, to distinguish a broad polyclonal rise from a narrow monoclonal spike.24

Low globulin

A low globulin generally means low antibodies. The first move is to look for a secondary cause, which is far more common than a primary immune deficiency:6

  • medications — long-term corticosteroids, antiseizure drugs, immunosuppressants, and especially anti-CD20 antibodies such as rituximab, which deplete the B cells that make antibodies;7
  • protein loss — nephrotic syndrome through the kidneys, or protein-losing enteropathy through the gut;
  • blood cancers — myeloma, lymphoma, or chronic lymphocytic leukemia, where the clone crowds out normal antibodies. This carries a real infection risk and sometimes justifies immunoglobulin replacement therapy.5

Only after those are excluded does a primary immunodeficiency — in adults, most often common variable immunodeficiency — come into view.6 An isolated, mildly low result with no history of recurrent infection should first simply be repeated.

Alpha-1 and alpha-2

A high alpha-1 almost always means inflammation, rising alongside CRP. A very low alpha-1 is rarer but far more consequential: it should prompt testing for alpha-1 antitrypsin deficiency, an inherited condition that leaves the lungs without their main protection against their own enzymes. It shows up as early emphysema — often before age 45, much worse in smokers — and sometimes as liver disease. It remains substantially underdiagnosed in the United States; a specific alpha-1 antitrypsin level followed by genotyping confirms it.910

A high alpha-2 points either to inflammation, or to nephrotic syndrome: the kidney leaks small albumin but retains the very large alpha-2 macroglobulin, producing the classic low albumin with high alpha-2 pattern. A low alpha-2 suggests hemolysis, with haptoglobin consumed and falling.1

Beta

The beta fraction carries transferrin, so it rises in iron deficiency — the body makes more carrier protein to capture scarce iron — and also in pregnancy, on estrogen, and with high cholesterol. A low beta occurs with malnutrition, liver disease, and protein loss.

"Beta-2 globulin" is not "beta-2 microglobulin." On electrophoresis, beta-1 and beta-2 are just the two subzones of the beta band — a matter of position on the tracing, not a specific disease. Beta-2 microglobulin is a completely different protein, ordered separately, used mainly in myeloma staging and kidney assessment. Seeing "beta-2" on an electrophoresis report does not mean beta-2 microglobulin was measured.811

Gamma

A high gamma has two very different shapes. A broad, dome-like rise without a peak — polyclonal hypergammaglobulinemia — is a normal immune reaction to chronic infection, autoimmunity, or liver disease, and is by far the more common. A narrow, sharp spike is a monoclonal protein, produced by a single plasma-cell clone, and calls for specialist evaluation with immunofixation and serum free light chains.11

A spike is not cancer. The most frequent cause is MGUS, present in roughly 3% of people over 50, which progresses to active disease at only about 1% per year — most people with a small spike simply need monitoring.12 Our SPEP guide walks through that reading in full.

A low gamma is the hypogammaglobulinemia described above.

What can affect your globulin level

Age (gamma drifts up over the years), pregnancy and estrogen (which raise the beta fraction), dehydration (which concentrates all proteins), a recent ordinary infection, and medications — corticosteroids, immunosuppressants, therapeutic monoclonal antibodies. There is no fasting requirement; if other tests on the same order require it, follow those instructions (fasting before a blood test).1

Recent research

  • The A/G ratio is firming up as a secondary prognostic marker. A 2026 meta-analysis of six studies and 2,107 patients undergoing liver-cancer surgery found that a low preoperative A/G ratio was associated with significantly worse overall and recurrence-free survival. The authors stress that prospective validation is still needed and that the optimal cutoff varies between studies — this is an adjunct, not a diagnostic test.3
  • Reading capillary electrophoresis is becoming less subjective. A 2024 study proposed five quantitative indexes — including a "sharpness index" for peaks — with reference intervals derived from 1,000 normal tracings in both the gamma and beta-2 zones, aiming to reduce disagreement between laboratories.8
  • Alpha-1 antitrypsin deficiency is no longer untreatable. Beyond standard augmentation therapy, several approaches are in trials: a recombinant alpha-1 antitrypsin–Fc fusion with an extended half-life tested in a phase 1 study, gene therapy, and small molecules that stop the protein aggregating in the liver. An international cohort is following hundreds of patients to identify biomarkers of progression.101314

Can I lower high globulin naturally?
There is no "anti-globulin diet." Globulins follow an underlying cause — inflammation, infection, liver disease, a gammopathy. Treating that cause is what normalizes them, and those decisions belong with your physician.
