What is vitamin B6?

Vitamin B6 is an umbrella term for several related molecules (pyridoxine, pyridoxal, pyridoxamine) supplied by food: poultry, fish, organ meats, potatoes, bananas, chickpeas and other legumes, and fortified cereals. Once absorbed, they are converted by the liver into the biologically active form, pyridoxal-5-phosphate (PLP). That is the form the lab measures in plasma as a marker of your B6 stores.21

PLP is an essential cofactor: it clips onto dozens of enzymes so they can work. It drives amino-acid (protein) metabolism, the production of neurotransmitters (serotonin, dopamine, GABA), the synthesis of hemoglobin (the oxygen carrier in red blood cells), and the recycling of homocysteine into cysteine. That wide range of jobs is why a deficiency can hit organs as different as the skin, the nerves, and the blood.24

Why the test is done

Low vitamin B6 (deficiency)

A low vitamin B6 signals a deficiency, and its features, when present, span several organs:410

  • skin and mucous membranes: seborrheic dermatitis, cheilitis (cracks at the corners of the lips), glossitis (a smooth, sore tongue);
  • nervous system: peripheral neuropathy (tingling, burning, numbness in the feet and hands), confusion, irritability, and — especially in infantsseizures (pyridoxine-dependent forms);
  • blood: anemia, classically microcytic or even sideroblastic, because PLP is needed to make hemoglobin.

The most common causes are chronic alcohol use, malabsorption (celiac disease, bowel surgery), kidney failure and dialysis, and certain drugs. Isoniazid (a TB medication) is the classic example: it neutralizes B6 and can cause neuropathy, which is precisely why pyridoxine is given alongside it to prevent nerve damage.5 One point that is easy to miss: PLP drops during inflammation, independent of diet. A low PLP in someone with a high CRP may reflect the inflammation itself rather than a true dietary shortfall — which is why the two are best read together.17

High vitamin B6 (excess): the supplement trap

A high vitamin B6 almost always comes from supplementation (multivitamins, "energy" drinks, standalone B6 pills), not from disease. The crucial point — rarely said — is that this excess is not harmless. In high doses over a prolonged period, pyridoxine causes a sensory neuropathy: tingling, numbness, unsteadiness, sometimes pain, which usually improve after stopping.64 In other words, too much B6 produces the same neurological signs as a deficiency — the textbook case of "more is not better." That is why health authorities set a tolerable upper intake level for B6.2

Vitamin B6 and cancer? Vitamin B6 is not a tumor marker. A low or high level says nothing about a tumor — it speaks to your diet, your medications, possible inflammation, or supplementation.

The B6–B9–B12 trio and homocysteine

Vitamin B6 is rarely read alone. With B9 (folate) and B12, it forms the trio that recycles homocysteine. A B6 deficiency can therefore contribute to a high homocysteine.
