What is procalcitonin?

Procalcitonin is the precursor of a hormone, calcitonin, normally made by the C-cells of the thyroid. In a healthy person, almost all of it is converted into calcitonin, so the blood level is very low (usually undetectable).9

That changes with a serious bacterial infection. Driven by bacterial toxins and inflammatory mediators, many tissues across the body (liver, lungs, gut, muscle) begin producing procalcitonin and releasing it into the blood. Understanding your results

High procalcitonin: causes

A high procalcitonin points first toward a systemic bacterial infection — the higher it climbs, the more likely the infection is severe and widespread.12 The main situations:

  • a serious bacterial infection: pneumonia, pyelonephritis (kidney infection), bacterial meningitis, catheter-related infection, peritonitis…;
  • sepsis or septic shock, where PCT reaches its highest values (often > 2, even > 10 µg/L);2
  • non-infectious causes worth knowing about: major surgery, major trauma, extensive burns, cardiogenic shock, severe pancreatitis, certain immunotherapies, or end-stage kidney disease (the kidneys clear PCT);67
  • in a newborn, a transient physiologic rise occurs in the first 48 hours of life, without infection;6
  • medullary thyroid carcinoma, which produces calcitonin and procalcitonin, can cause a chronic elevation (a rare, special case).

Does a high PCT mean I need antibiotics? Not automatically. A high PCT raises the probability of a bacterial infection but does not prove it: it is one argument among several, weighed against the exam, CRP, ESR, white blood cells, and microbiology. PCT and cancer? Apart from the very specific case of medullary thyroid carcinoma, a high PCT is not a tumor marker: it speaks to infection and systemic inflammation, not a tumor.

Low procalcitonin

A low or normal PCT is reassuring against a systemic bacterial infection: it makes sepsis unlikely and, in lower respiratory infections, argues against prescribing antibiotics.45 But a low PCT does not rule out:
