What is SHBG?

SHBG is a transport protein made by the liver and released into the blood. Like a shuttle, it grabs sex hormones — mainly testosterone and estradiol — and carries them around. In the bloodstream, these hormones travel in three forms: a portion tightly bound to SHBG (inactive, held in reserve), a portion loosely bound to albumin, and a small free fraction. The "free + albumin-bound" total is the bioavailable hormone — the part that can actually reach and act on tissues.12

So SHBG works as a regulator: by holding onto some of the hormone, it decides how much is truly active. High SHBG sequesters more and lowers the free fraction; low SHBG releases more and raises it. That's why two people with the same total testosterone can have very different free testosterone, depending on their SHBG.

Why the test is done

Low SHBG

A low SHBG releases more hormone, so free testosterone rises. The main causes are metabolic and hormonal:756

  • obesity, insulin resistance, type 2 diabetes, metabolic syndrome: excess insulin suppresses liver production of SHBG — the most common reason;95
  • polycystic ovary syndrome (PCOS) in women: low SHBG amplifies androgen excess (acne, hirsutism);6
  • hypothyroidism (an underactive thyroid);
  • androgens or anabolic steroids, corticosteroids, and, more rarely, acromegaly or Cushing's syndrome.

In men, low SHBG can make total testosterone look "low" even when free testosterone is normal — which is exactly why the calculation matters. In women, it points to an insulin-resistant profile worth exploring.

High SHBG

A high SHBG holds onto more hormone, so the free fraction falls. Main causes:89

  • hyperthyroidism (an overactive thyroid), which strongly drives SHBG synthesis;
  • liver disease, especially cirrhosis;
  • estrogen exposure: combined birth control pills, hormone therapy, and pregnancy (SHBG rises substantially);
  • older age (it tends to rise in older men), anorexia / undernutrition, and some anti-seizure drugs.

In men, a high SHBG can "mask" low free testosterone despite a normal total — again, calculating free testosterone settles it.

The SHBG–testosterone–estradiol triangle

SHBG is interpreted as a triangle with the hormones it carries. Low SHBG with a normal total testosterone can mean high free testosterone (useful in a woman with signs of androgen excess).
