Lactate (the dissolved form of lactic acid) is a small molecule your cells make continuously when they turn sugar into energy. Its level climbs mainly when a tissue runs short of oxygen or blood flow — which is why the lactate blood test is a front-line alarm signal in the emergency room and the ICU, above all in sepsis and shock. What is lactate?

Lactate is the dissolved form of lactic acid. It comes from glycolysis, the chain of reactions that breaks down glucose (sugar) for energy. At the end of that chain, a molecule called pyruvate is converted into lactate. Contrary to a stubborn myth, this does not happen only when oxygen is lacking: your muscles, brain, red blood cells, and gut make lactate continuously, including at rest and with plenty of oxygen. That lactate is then recycled — the liver (and, to a lesser extent, the kidneys) turns it back into glucose or burns it for fuel. Scientists call this the "lactate shuttle" between cells that produce it and cells that consume it.1

So the lactate on your report reflects a balance: what your body makes minus what it clears. It rises when production surges (cells short of oxygen, violent exercise) or when clearance is impaired (a failing liver). That is what makes it a valuable signal — but one that must always be read in context.2

Lactate is not the same as acidosis. Lactic acidosis means a very high lactate has made the blood too acidic. But you can have a mildly high lactate without acidosis (after exercise, say). Lactate is always read together with the pH and the bicarbonate from a blood gas.

Why the test is done

Lactate is not part of a routine wellness panel. It is mostly ordered in the emergency department or the ICU, to gauge how sick someone is and to track them over time:26

  • to detect and follow shock or sepsis — a high lactate signals that organs are getting too little oxygen; it is one of the severity criteria for septic shock;4
  • to guide resuscitation — the test is repeated to confirm the lactate is coming down with treatment (its "clearance"); a falling lactate is a good sign;42
  • to work up an unexplained metabolic acidosis, alongside the electrolyte panel, the bicarbonate, and the anion gap;
  • to monitor high-risk situations: resuscitated cardiac arrest, poisoning, liver failure, or suspected lactic acidosis from certain drugs.7

Lactate is measured on venous or arterial blood (often within a blood gas), and increasingly at the bedside using portable point-of-care analyzers (see below).8

How the test is done

Lactate is a standard blood draw, or it may be pulled from an arterial blood gas. There is no need to fast — in an emergency it is drawn immediately, at any hour. Because it swings with muscle activity, you should be at rest and avoid intense exercise just beforehand. The truly critical part is not fasting but technique: a tourniquet left on too long, a clenched fist, or a tube that sits at room temperature all push the number up (red cells keep making lactate in the tube). Normal ranges

Below are indicative adult reference values at rest. They vary by lab, by sample type (venous vs arterial), and by clinical state — always compare against the range printed on your report.

Units: most labs report mmol/L; U.S. labs may use mg/dL (1 mmol/L ≈ 9 mg/dL). During intense exercise, lactate can spike transiently to 10–20 mmol/L in a healthy person with no danger at all — that is physiological.12 Only your clinician can interpret the number in your context.

A low lactate carries no worrying meaning — it is the normal, desired state. There is no "low-lactate disease," and a value at the bottom of the range needs no treatment.

High lactate: causes

A high lactate (hyperlactatemia) is usually sorted into two broad types.32

Type A lactic acidosis (hypoxic) — the most common and most serious. Tissues are short of oxygen or blood flow:

  • sepsis and septic shock (severe infection) — the flagship cause in the ICU; a high lactate that fails to fall carries a poor prognosis;42
  • shock of any kind (hemorrhagic, cardiogenic), cardiac arrest, circulatory failure;
  • severe hypoxemia, profound anemia, or ischemia of an organ (bowel, for example);
  • intense muscle exertion or seizures — a large but transient surge, harmless, that corrects itself with rest.1

Type B lactic acidosis (non-hypoxic) — circulation is fine, but lactate production or clearance is disturbed:

  • liver failure — the liver can no longer clear enough lactate;
  • drugs and toxins — above all metformin (a diabetes drug), which can rarely cause lactic acidosis, especially with kidney impairment or dehydration; alcohol and other agents also contribute;7
  • mitochondrial diseases and certain inherited metabolic disorders;
  • some advanced cancers (tumor cells over-produce lactate — see below).5

In practice, a high lactate is never a diagnosis on its own: it tells you an organ is struggling or a metabolism is disturbed, and it demands a search for the cause. Management is decided by your clinician, weighing the rest of the workup — the electrolyte panel, bicarbonate, sodium, kidney and liver function, and sometimes procalcitonin when infection is suspected.

Lactate and cancer? Tumor cells produce a lot of lactate (the "Warburg effect"), and research is actively probing lactate's role in inflammation and cancer.5 But lactate is not a tumor marker and is not used to screen for cancer: a mildly high lactate on a blood test does not mean you have a tumor.

The exercise-lactate myth

This is the most persistent misconception. The lactate you make during exercise (running, lifting, HIIT) is normal, useful, and transient — it is not a "toxic waste product" and not the direct cause of muscle soreness. It is quickly recycled as fuel by the heart, liver, and resting muscle.1 Exercise lactate has nothing to do with the lactate measured in shock: in one case the body is running at full throttle, in the other organs are in distress. Do not conflate the two.

What affects your lactate

Many things move the lactate level: muscle exertion just before the draw, stress, hyperventilation, liver and kidney function (clearance), oxygenation, certain medications (metformin, epinephrine, some antiretrovirals), and alcohol.37 But the sneakiest factor is technical: a tourniquet left on too long, clenching the fist, or a tube that takes too long to reach the lab all falsely raise the result, because red cells keep making lactate in the tube. A good draw uses no prolonged tourniquet, an appropriate tube, and rapid, chilled processing.2 Always mention your medications and any recent exercise — they change the interpretation.

  • Lactate clearance guides sepsis resuscitation. The international Surviving Sepsis Campaign 2021 guidelines suggest guiding resuscitation in septic shock by working to lower an elevated lactate: a falling lactate reflects better organ perfusion.4 A dedicated review cautions, though, that lactate is an imperfect marker — sensitive but not specific — best read alongside the clinical picture rather than alone.2 (Evans L et al., Intensive Care Med, 2021 — DOI.)
  • Don't chase the number blindly. A post hoc analysis of the ANDROMEDA-SHOCK trial found that a strategy driven by lactate alone was associated with higher mortality in patients whose peripheral perfusion (capillary refill time) was already normal — evidence that lactate should be combined with other markers, not pursued in isolation.10
