What is haptoglobin?

Haptoglobin (often abbreviated Hp) is a glycoprotein in blood plasma, made mostly by the liver (and a little by the lungs). Its main task is to bind tightly to free hemoglobin — the hemoglobin that escapes when red blood cells rupture. Every day a small fraction of red cells is renewed and releases a little hemoglobin; haptoglobin captures it immediately, forming a haptoglobin–hemoglobin complex. A macrophage receptor called CD163 recognizes that complex, takes it in, and recycles it. This keeps free hemoglobin and its oxidative iron from damaging blood vessels and kidneys.17

The logic follows: when many red cells break at once, haptoglobin is consumed faster than the liver can replace it, and the level drops. That's why a low haptoglobin is, above all, a signal of hemolysis — not a disease in itself.1

Why the test is done

Haptoglobin's value lies in its sensitivity: it is often the first parameter to collapse in hemolysis, even before anemia becomes obvious.458 Outside that setting, it may also be checked as part of an inflammation work-up, since it rises in inflammatory states.2

Good to know: haptoglobin is naturally low in newborns (the liver is still maturing) and rises over the first months of life. An undetectable value points strongly toward hemolysis or a genetic cause. A high value, by contrast, is non-specific (usually inflammation). Low haptoglobin: the hemolysis signal

A low haptoglobin should first raise the question of hemolysis: red cells are being destroyed faster than normal, and the released hemoglobin "uses up" available haptoglobin.14 Leading causes:4

  • autoimmune hemolytic anemia (the immune system attacks red cells), confirmed by a positive direct antiglobulin (Coombs) test;9
  • mechanical hemolysis: a mechanical heart valve, thrombotic microangiopathy (with schistocytes on the smear), extracorporeal circulation, or certain procedures;
  • a transfusion reaction or acute intravascular hemolysis;
  • inherited red-cell disorders: sickle cell disease, thalassemias, hereditary spherocytosis, G6PD deficiency, paroxysmal nocturnal hemoglobinuria;
  • less often, hypersplenism (an overactive spleen) or some infections (malaria).

But a low haptoglobin does not always mean hemolysis. It can also reflect:26

  • liver disease (cirrhosis, liver failure): the liver makes less haptoglobin, so the level falls without red-cell destruction;
  • congenital anhaptoglobinemia: some people carry a complete deletion of the haptoglobin gene (the HPdel allele), found especially in East and Southeast Asian populations; their haptoglobin is permanently undetectable, with no hemolysis at all.6

The pattern to know. Haptoglobin is read with its "neighbors": in hemolysis you expect low haptoglobin together with high reticulocytes (the marrow makes more red cells to compensate, often > 120 × 10⁹/L), high LDH (an enzyme released by destroyed cells), and high indirect bilirubin (a hemoglobin breakdown product). That combination, plus a look for schistocytes on the peripheral smear, makes the diagnosis — not haptoglobin alone.4

High haptoglobin

A high haptoglobin is non-specific and rarely worrying by itself. Haptoglobin is an acute-phase protein: it rises during infections, chronic inflammation, some cancers, after surgery, or with corticosteroids.2 It climbs for the same reasons as CRP or the sed rate, without pointing to any one disease.

The trap not to miss. Because inflammation raises haptoglobin while hemolysis lowers it, someone who has both at once can show a falsely "normal" value. Inflammation can therefore mask hemolysis. That's exactly why clinicians never trust haptoglobin in isolation — they cross-check it against LDH, reticulocytes, bilirubin, and the clinical picture.21

A 2025 clinical review reaffirms that the diagnosis of hemolysis rests on simple parameters — haptoglobin (the most sensitive), LDH, and indirect bilirubin — with reticulocytes typically above 120 × 10⁹/L, and stresses the smear and Coombs test to sort out the cause.4 An older, foundational study had already put numbers on it: low haptoglobin has about 83% sensitivity and 96% specificity for hemolytic disease.5
  • A better-understood hemoglobin "clean-up" system. Work on the haptoglobin–hemopexin–CD163 axis has clarified how the body neutralizes toxic free hemoglobin and heme, and has opened therapeutic avenues (haptoglobin or hemopexin "drugs") in sickle cell disease, massive transfusion, and brain hemorrhage.17
  • Hp phenotypes and disease risk. The human haptoglobin gene is polymorphic (Hp1-1, Hp2-1, Hp2-2). The Hp2-2 phenotype, with lower antioxidant capacity, has been linked to more subclinical vascular disease (carotid plaque), especially in people with diabetes.102
