Low ceruloplasmin and low copper: the Wilson disease clue
The single most important pattern to know is Wilson disease. It's a genetic condition (a mutation in the ATP7B gene) that stops the liver from clearing copper into bile. Copper then builds up in the liver, and later the brain, eyes, and other organs — causing hepatitis, cirrhosis, tremor, dystonia, or psychiatric symptoms, often in a teenager or young adult.14
The paradox trips people up: in Wilson disease, ceruloplasmin is usually low and total serum copper is usually low too. How can a copper overload produce a low blood copper? Because measured total copper is mostly the copper bound to ceruloplasmin: the diseased liver makes less ceruloplasmin, so total copper falls with it. But the free copper (not bound to ceruloplasmin) is increased — and that's the toxic fraction. Copper deficiency
