What is renin?
Renin is an enzyme made by specialized cells in the kidney, clustered in the juxtaglomerular apparatus. The kidney releases it mainly when it senses a drop in pressure, a drop in blood volume, or a drop in the sodium reaching it. Once in the blood, renin converts a liver protein, angiotensinogen, into angiotensin I, which is quickly turned into angiotensin II by angiotensin-converting enzyme (ACE).1
Angiotensin II is a powerful vasoconstrictor (it tightens blood vessels) and it drives the adrenal gland to make aldosterone, the hormone that tells the kidneys to hold onto salt and water and excrete potassium. Why the test is done

How the test is done

Understanding your results
Renin is always read with aldosterone. The key number is usually the aldosterone-to-renin ratio (ARR): a high ratio (high aldosterone with low renin) points toward primary aldosteronism, whereas renin and aldosterone both high suggest a kidney or vascular cause instead.53 ARR cutoffs depend on the units and assay your lab uses.8
Low renin
A low (suppressed) renin most often accompanies salt retention or an excess of aldosterone or other mineralocorticoids. Main causes:57
- primary aldosteronism (an adrenal adenoma or hyperplasia): excess aldosterone suppresses renin — the diagnosis you don't want to miss, flagged by a high ARR;5
- excess salt or mineralocorticoids (including large amounts of licorice, some steroids);

High renin
A high renin usually reflects the kidney trying to raise a pressure or volume it judges too low. Main causes:13

- dehydration or hypovolemia (blood loss, salt loss, salt-free diet);
- diuretics, which drive off salt and water;

- pregnancy, where the RAAS is physiologically activated;
- RAAS drugs: ACE inhibitors and ARBs, along with mineralocorticoid-receptor antagonists (spironolactone), all raise renin — an expected effect you need to account for.5
