High Alkaline Phosphatase (ALP): Liver or Bone?

A high alkaline phosphatase comes from the liver or from bone. GGT usually sorts it out, and growth, pregnancy and healing fractures raise ALP normally.

Your lab report shows an alkaline phosphatase above the range printed next to it — your report may call it ALP or ALK PHOS. That number really only asks one question, and it is a simple one: is this enzyme coming from your liver, or from your bones? Everything else follows from the answer — which tests your clinician orders next, how fast, and how much concern the result deserves. This page answers that question. First: is this really abnormal?

The upper limit is not the same everywhere — or for everyone. U.S. labs publish different intervals for the same enzyme. One large national laboratory reports 47–123 IU/L for men and 41–116 IU/L for women aged 21–50, and 49–135 IU/L for women aged 51–80.1 Others print 44–147 U/L, and some 30–120 U/L, for both sexes.2 A man at 130 U/L is therefore "high" on one report and unremarkable on another. The American College of Gastroenterology says so plainly: clinicians "may rely on local lab ULN ranges for alkaline phosphatase."3 Compare yourself to the limit on YOUR report, not to a number on a website — this one included.

A result just above the line is not a disease. Reference intervals are built to contain about 95% of healthy people, so by construction one healthy person in twenty falls outside without anything being wrong. U.S. reference texts put a figure on it: an elevation of less than 50% above the upper limit generally needs no extensive work-up, only periodic repeat testing.4

The draw itself can add a few units. One rare, verifiable detail is worth knowing: intestinal ALP enters the blood after a fatty meal, far more so in people of blood group O or B who are secretors — first published in Nature in 1966, still listed in U.S. references today.564 That fraction can persist for up to 12 hours, which is one reason a fasting draw is preferred.4 The effect on total ALP stays modest: after a standardized meal the rise is statistically measurable but not clinically significant.7 Breakfast will not explain a clearly abnormal number — but it can tip a borderline one over the line (see fasting before a blood test and blood type).

Liver or bone? The split your clinician makes first

The rule fits on one line:

  • High ALP + high GGT → hepatobiliary origin (bile flowing poorly, which is called cholestasis);
  • High ALP + normal GGT → look at bone.

This is recommendation 13 of the ACG guideline, word for word: "An elevation of alkaline phosphatase should be confirmed with an elevation in GGT."3 GGT is present in the liver but not in bone, which is exactly what makes it useful here.5

But you should know what the rule is worth. In the guideline's own text it is graded "strong recommendation, very low level of evidence" — expert consensus, not a validated test with a published sensitivity and specificity.3 Three limits follow, and they matter:

  1. A high GGT does not prove the ALP came from the liver. GGT rises with alcohol, enzyme-inducing drugs, fatty liver and excess weight, so two markers can be high side by side without being related. The same recommendation states that, "given its lack of specificity for liver disease," GGT should not be used as a standalone screening test for liver disease.3 That is the subject of the high GGT page.
  2. A normal GGT does not entirely clear the liver. Cholestasis with a normal GGT is real — progressive familial intrahepatic cholestasis, bile-acid synthesis defects, galactosemia — but it belongs almost entirely to newborns and infants.8 In an adult, a normal GGT makes a bone source distinctly more likely.
  3. The rule points; it does not conclude. It decides which work-up comes next: abdominal ultrasound on one side, calcium, phosphorus, 25-OH vitamin D and PTH on the other.

The causes, from most common to rarest

1. Physiological causes — the most frequent, and the most reassuring

Growth. In children and teenagers, bone under construction manufactures ALP in bulk, and normal values run two to four times adult ones. U.S. pediatric reference intervals, in IU/L:1

A 13-year-old boy at 400 IU/L is inside his own range, while the same number in his father would call for a work-up. Girls reach adult values earlier. If the result belongs to a child, check that the range applied is the pediatric one (see blood draws in babies and children).

The third trimester of pregnancy. The placenta makes its own ALP isoenzyme, and levels climb late in pregnancy.43 A measured order of magnitude: in a cohort of 11,853 women sampled on admission for labor, the median ALP was 149 U/L, with a 5th–95th percentile range of 89–248 U/L.9 More than half of women at term therefore exceed the non-pregnant upper limit — and it means nothing. That same study found women in the highest ALP quartile had a lower risk of preterm birth than those in the lowest.9

A healing fracture. Repairing bone raises ALP temporarily, "rising after 1 week and persisting for up to 3 months" — common, entirely benign, and routinely forgotten when the history is taken.5

In infants and toddlers, transient hyperphosphatasemia. A well-described entity: an isolated ALP elevation — reported at up to 30 times the age-specific upper limit — with a normal exam and a normal rest of the panel, often after a mild viral illness.4 It resolves spontaneously in 2 to 3 months and needs no treatment; recognizing it prevents an entirely unnecessary work-up.10

2. The liver and bile ducts

In an adult with a high GGT alongside, a raised ALP reflects cholestasis — impaired bile flow — and this is the enzyme's main clinical use.411 Roughly by frequency in general practice: medications, including drug-induced liver injury from over-the-counter products, herbals and dietary supplements — a genuinely American problem, since the share of cases attributed to supplements in the NIH-funded Drug-Induced Liver Injury Network rose from 7% to 20% over a decade;12 gallstones and other bile-duct obstruction; steatotic and chronic liver disease (fatty liver, hepatitis, cirrhosis); then primary biliary cholangitis (PBC), sclerosing cholangitis, and infiltrative liver disease.34 The result is read with bilirubin and the transaminases ALT and AST: the whole liver panel concludes, never ALP alone.11

3. Bone

Vitamin D deficiency: this needs serious qualifying. Everywhere you read that low vitamin D raises ALP. It does — once it has produced osteomalacia, poorly mineralized bone. But osteomalacia is rare. A service evaluation of 25,379 adults who had a 25-OH vitamin D measured found biochemical osteomalacia in 0.5% and clinically confirmed osteomalacia in only 0.1% — about 2 to 3 cases a year in a population of half a million. None of the untreated confirmed cases had a vitamin D above 10 ng/mL.13 Low vitamin D is banal; its being the explanation for your high ALP is not.

Primary hyperparathyroidism with skeletal involvement raises ALP moderately, and is picked up on the calcium and phosphate panel — a calcium above roughly 10.2 mg/dL with an inappropriately normal or high PTH.4

Paget's disease of bone is the textbook case of a high ALP "with nothing else." Many patients are asymptomatic and are identified precisely through an unexplained ALP elevation.14 NIAMS lists the ALP blood test among the standard tests used to diagnose it, with X-ray as the most common and a bone scan to map the extent; bisphosphonates are the mainstay and bring ALP back down. The disease is uncommon under age 40.1514

Less often: renal osteodystrophy, hyperthyroidism (not hypothyroidism), osteogenic sarcoma, and bone metastases.4

4. The rare, named without drama

Bone metastases, primary bone tumors and lymphomas can raise ALP.4 Two things are true at once: these causes exist, and they come last on a list ordered by real-world frequency — behind growth, pregnancy, fractures, medications and gallstones. ALP is not a cancer screening test.

What should send you to a doctor without waiting

  • Jaundice (yellow skin or eyes), dark urine, pale stools, or diffuse unexplained itching.
  • Severe pain in the right upper abdomen, especially with fever or chills.
  • Localized, constant bone pain, a deformed bone, or a fracture from a minor injury.
  • Unintended weight loss or a general decline.

Outside these situations, a moderately high ALP is not an emergency — it is a scheduled appointment.

What your doctor will do next

  1. Confirm it — repeat the test, with GGT.3
  2. Settle liver versus bone — GGT, bilirubin and transaminases on one side; calcium (mg/dL), phosphorus (mg/dL), 25-OH vitamin D (ng/mL) and PTH on the other.
  3. If the source is hepatic — a right upper quadrant ultrasound is the recommended first imaging test. If it is normal, intrahepatic causes are pursued: PBC via anti-mitochondrial antibodies, sclerosing cholangitis via MR cholangiography with IgG4, and drug- or supplement-induced injury by reviewing everything you take.3 NIDDK notes that PBC may be diagnosed on anti-mitochondrial antibodies plus a higher-than-normal ALP even with no other symptoms.16
  4. If the source looks skeletal — targeted X-rays, and a bone scan if Paget's disease is suspected.15

ALP isoenzymes: when measuring them actually helps

A lab can separate the isoenzymes — bone, liver, intestinal, placental — by electrophoresis, heat inactivation or a direct immunoassay for the bone fraction.4 This is worth doing when doubt persists after the first sort: an isolated elevation that will not explain itself, or monitoring a known bone disease. It has no first-line value — for a mildly high ALP in an adult, GGT and the calcium-phosphate panel settle it almost every time — and heat inactivation in particular is imprecise, because the liver and bone profiles overlap.4

What this result does NOT mean

  • It is not a cancer test. The largest elevations belong overwhelmingly to benign disease: ALP reaches 4 times the upper limit or more in up to 75% of people with cholestasis, and Paget's disease and osteomalacia produce the biggest bone elevations.4 What magnitude tells you is that a number needs an explanation — not what the explanation is.
  • It is not "alcohol damage." That reputation belongs to GGT, and even there imperfectly.3
  • It is not osteoporosis. ALP levels "are usually normal in osteoporosis."5
  • In a teenager or a pregnant woman, it is usually nothing at all — it is physiology.19
  • A low ALP is a different subject entirely — zinc or magnesium deficiency, hypothyroidism, hypophosphatasia — and is covered on the marker guide.

Should you repeat the test, and when?

For a moderate, isolated, symptom-free elevation, the usual course is to recheck at a distance — a few weeks to three months — adding GGT, rather than stacking up investigations.34 That delay is not passivity: it gives transient causes time to disappear. ALP has a half-life of about 7 days, and can stay elevated for up to a week even after a biliary obstruction has been relieved, so an immediate recheck can mislead.4 Transient hyperphosphatasemia of infancy normalizes in 2 to 3 months;10 fracture ALP falls with healing; pregnancy ALP disappears with the placenta. By contrast, an elevation that is marked, rising between two draws, or accompanied by any of the warning signs above is not watched — it is investigated.

Frequently asked questions

When should I worry about high alkaline phosphatase?
Not because of the isolated number, but because of its size, its trend and your context. A moderate, isolated elevation in a teenager, a pregnant woman, or after a fracture is usually physiological, and an elevation under 50% above the upper limit generally warrants monitoring rather than testing.4 Jaundice, dark urine, constant bone pain or weight loss justify a prompt visit.
What ALP level counts as high?
Whatever exceeds the upper limit on your report. As a guide for adults: about 47–123 IU/L in men and 41–116 IU/L in women aged 21–50, with women aged 51–80 running to 135; other U.S. labs print 44–147 or 30–120.12 The ACG explicitly tells clinicians to use their local range.3
High ALP with normal GGT — where does that come from?
In an adult, it points to bone: residual growth, a healing fracture, osteomalacia, hyperparathyroidism, Paget's disease. The rule is a strong recommendation resting on a very low level of evidence, so it directs the work-up rather than replacing it.3
Is high alkaline phosphatase in pregnancy dangerous?
No — it is expected in the third trimester, because the placenta makes its own ALP. In a cohort of 11,853 women sampled at delivery the median was 149 U/L, and a higher level went with a lower risk of preterm birth.9
My child's ALP is far above the adult range — should I be worried?
Usually not. Between ages 1 and 15, U.S. reference intervals run up to 369–435 IU/L depending on age and sex.1 Check that the range applied is pediatric. In infants and toddlers, transient hyperphosphatasemia can push ALP several times higher still and resolves on its own in 2 to 3 months.104
Can supplements raise ALP?
Yes. Herbals and dietary supplements are a recognized cause of liver injury in the U.S.: their share of cases in the NIH-funded DILIN registry rose from 7% to 20% over a decade.12 Bring every bottle to the appointment.
How do I lower a high alkaline phosphatase?
There is no remedy for ALP itself; you treat the cause. Correcting a vitamin D deficiency, stopping the responsible drug or supplement, relieving a biliary obstruction, or treating Paget's disease brings the number down.1514 No diet or supplement lowers ALP on its own.
