High Creatinine Levels: Causes, eGFR and When to Worry

A high creatinine is read through the eGFR, not alone. Non-kidney causes, US mg/dL ranges, the race-free CKD-EPI 2021 equation and next steps.

Your creatinine came back above your lab's upper limit, and the line may be flagged in red or marked H. First: is this really abnormal?

The number to read is not the one your lab flagged

Creatinine is a waste product your muscles make continuously and your kidneys clear. Its blood level therefore depends on two things: how much your kidneys remove, and how much your muscles produce. More muscle means more creatinine — with no kidney filtering any worse.

That is why your lab converts it into an estimated glomerular filtration rate (eGFR), in mL/min/1.73 m², using creatinine plus age and sex. An example beats an argument. With exactly the same creatinine, 1.4 mg/dL, flagged high in both cases, the race-free CKD-EPI 2021 equation gives:1

The reverse is also true. That same 78-year-old woman with a creatinine of 1.0 mg/dL — sitting comfortably inside the 0.5–1.1 mg/dL female range some labs use, so printed in plain black — has an eGFR near 58: a stage G3a that nobody flagged. The borderline zone, and cutoffs that move between labs

A result just over the line is not a disease. Reference intervals are built so that a small share of perfectly healthy people fall outside them by construction. And those cutoffs genuinely differ from lab to lab with the assay: MedlinePlus gives roughly 0.7–1.3 mg/dL for men and 0.5–0.95 for women, while StatPearls gives about 0.6–1.2 and 0.5–1.1.23 Compare yourself to the limit printed on your report, not to a number on a website.

What you did before the draw matters

Three very ordinary situations push creatinine up by a tenth or two of a mg/dL without touching your kidneys:

  • A cooked-meat meal. Cooking converts the creatine in meat into creatinine, which you absorb directly. In the reference study, median creatinine rose from 0.91 to 1.14 mg/dL (80.5 to 101.0 µmol/L) one to two hours after a meat meal, and the eGFR computed on that sample fell from 84 to 59.5 mL/min/1.73 m². Cystatin C, measured on the same samples, barely moved.4 In patients with diabetes-related kidney disease, 6 of 16 at stage G3a were misclassified as G3b on a post-meal sample; the effect is gone after 12 hours of fasting.5
  • Dehydration. Heat, diarrhea, vomiting, diuretics: the blood concentrates and filtration dips temporarily.
  • Hard exercise, or creatine supplements. Creatine converts spontaneously into creatinine, so supplementing raises the blood level with no kidney dysfunction — controlled trials do not support the claim that creatine at recommended doses damages healthy kidneys.6

The causes, most common to rarest

1. High muscle mass. A young, athletic man at 1.3–1.5 mg/dL with a normal or barely reduced eGFR, a clean panel and no albumin in the urine is the single most common version of an "abnormal" creatinine with no disease behind it.

2. The pre-draw causes above — meat, dehydration, creatine, exertion — which account for a large share of isolated, reversible elevations.

3. Chronic kidney disease (CKD). The most common underlying cause in the general population: CKD affects more than 1 in 7 US adults, about 35.5 million people, and as many as 9 in 10 who have it do not know. About 1 in 3 adults with diabetes and 1 in 5 with high blood pressure have it.7 It climbs slowly, without symptoms, for years — which is why it is found on routine blood work. Markers that travel with it: BUN, potassium, bicarbonate, phosphate.

4. A "prerenal," reversible drop in filtration. The kidneys are not diseased — they are simply receiving less blood. Stomach flu, a heat wave, diuretics, an NSAID stacked on blood-pressure medication. Creatinine returns to its usual level once the cause is fixed.

5. Drugs that block creatinine's exit without harming the kidney. This is the least-explained cause and one of the most useful. Part of your urinary creatinine never goes through the glomerular filter at all: it is actively secreted by the kidney's tubular cells, entering through the transporter OCT2 and leaving through MATE1 and MATE2-K. Several drugs block that door, and blood creatinine rises while filtration has not changed at all.8 Documented offenders include trimethoprim (in trimethoprim–sulfamethoxazole), cimetidine, pyrimethamine, HIV drugs such as dolutegravir and cobicistat, the antiarrhythmic dronedarone, fibrates, corticosteroids, and several classes of targeted cancer agents — tyrosine kinase inhibitors, PARP inhibitors, CDK4/6 inhibitors.9 The rise appears within days of starting, stays modest, then plateaus — but it can be large enough to cross the thresholds that define acute kidney injury, producing a false read of kidney damage.8 The test that settles it is cystatin C, a filtration marker these transporters do not touch.9 ⚠️ Never stop a prescription on your own on this basis: sorting true from false is the prescriber's job.

6. An expected dip when starting certain drugs. ACE inhibitors, ARBs and SGLT2 inhibitors lower eGFR by a few points at initiation. That is not kidney injury and is not a reason to stop.10

7. A urinary blockage — an enlarged prostate, a stone, more rarely a compressing mass. It usually comes with trouble urinating, and an ultrasound sees it.

8. Rarer: disease of the kidney itself — glomerulonephritis, drug-induced interstitial nephritis, myeloma — or rhabdomyolysis, massive muscle breakdown after a crush injury, extreme exertion or certain drugs, which raises creatinine and CK together.

What should send you to a doctor promptly

These warrant a prompt call rather than waiting for your next appointment:

  • you are urinating much less than usual, or not at all;
  • swelling appears (ankles, puffy eyelids in the morning), or you gain several pounds in a few days;
  • you are short of breath at rest or lying flat;
  • there is blood in your urine, or persistently foamy urine;
  • nausea or vomiting that will not stop, unusual drowsiness or confusion;
  • your creatinine has jumped sharply versus a recent panel, especially alongside vomiting, diarrhea, extreme heat, or a newly started medication.

Outside these situations, a moderately high creatinine is handled at an ordinary office visit.

What your doctor will do next

Acute or chronic: only time tells them apart

These two are different diseases with different outlooks, and a single measurement cannot distinguish them.

  • Acute kidney injury (AKI) is defined by rapid change: a rise in creatinine of at least 0.3 mg/dL within 48 hours, or to at least 1.5 times a known or presumed baseline within the prior 7 days, or a fall in urine output.11 It is often reversible when the cause is treated.
  • Chronic kidney disease is defined by duration: an abnormality of kidney structure or function — eGFR below 60, or albumin in the urine, or an imaging abnormality — present for more than 3 months.10

So today's result, alone, says neither. What is needed is a prior value to compare against, or a repeat later on.

The three tests that almost always follow

  1. A repeat creatinine, drawn under decent conditions (see below).
  2. A urine albumin-to-creatinine ratio (uACR) on a single urine sample. This is the most frequently skipped test in the whole work-up, even though it weighs as heavily as eGFR in the prognosis. The abnormal threshold is 30 mg of albumin per gram of creatinine.10 See urine albumin.
  3. An ultrasound of the kidneys and urinary tract, to check kidney size and rule out obstruction or cysts.

Often added: an electrolyte panel, a blood glucose or A1c, and a blood-pressure reading — diabetes and hypertension being the two leading causes of kidney disease. The whole set is covered in kidney function tests.

The stages, if the eGFR stays low

⚠️ An eGFR of 60–89 is not kidney disease on its own: it becomes so only if albuminuria or another abnormality accompanies it for more than 3 months.1012 This is a common misreading, and a source of needless worry.

What this result does NOT mean

  • It is not a cancer marker. Creatinine reports on kidneys and muscle. It screens for no tumor.
  • It does not mean "kidney failure." That term describes a durably low eGFR, not a number above a cutoff on one particular morning.
  • It is not irreversible. Dehydration, a meal, a drug, a workout: the most common causes correct themselves and the value returns to your baseline.
  • It does not mean dialysis. Dialysis belongs to stage G5. Between a slightly high result and that point there are usually decades.
  • It is not a matter of race — and in the US that question is settled. The older CKD-EPI 2009 equation multiplied eGFR by 1.159 for patients recorded as Black, producing a result about 16% higher for identical creatinine. That coefficient is gone. Race is a self-reported social category, not a biological variable, and applying the coefficient overestimated measured GFR in Black participants — by a median of 3.7 mL/min/1.73 m² in the validation data — which could delay diagnosis, nephrology referral and transplant waitlisting.113 In 2021 the NKF-ASN Task Force recommended "immediate implementation of the CKD-EPI creatinine equation refit without the race variable in all laboratories in the United States," and the NIDDK now lists the race-free CKD-EPI 2021 equations as the standard.1314 KDIGO 2024 endorses race-free estimation as well.10 (European bodies reached a different technical answer, favoring other race-free equations for European populations — a live debate there, but not one that affects a US report.) If your lab report is a few years old and carries two eGFR values, one "African American," it is out of date: ask for it to be recalculated.

Should you repeat the test, and when?

Yes, in nearly every case — and it is often the only thing to do right away.

  • Modest elevation, no symptoms: a repeat within 2 weeks to 3 months is enough. Since chronicity is defined over 3 months, that is the interval that lets anyone conclude anything.10
  • Steep rise or an acute context (vomiting, heat wave, a new drug): a repeat within days, on medical advice.
  • Under what conditions? Well hydrated, with no meat-heavy meal and no hard workout in the previous 12 hours, and preferably at the same lab — changing labs means changing assay and cutoffs.5 Fasting is not required, but it removes the doubt (see fasting before a blood test).
  • How long to come back down? Hours after a meat meal, days once dehydration is corrected, days to weeks after stopping a drug that blocks tubular secretion.9

Frequently asked questions

At what creatinine level should I worry?
There is no universal number, and that is the honest answer: 1.4 mg/dL corresponds to an eGFR near 69 in a 30-year-old man and near 39 in a 78-year-old woman. Look at the eGFR, its trend, and your albuminuria — not at the mg/dL.
Does high creatinine mean kidney failure?
Not from one draw. Chronic kidney disease requires an eGFR under 60 (or another kidney abnormality) for more than 3 months. A single result cannot make that diagnosis.10
How can I lower my creatinine?
You do not treat a number, you treat its cause: normal hydration, stopping self-prescribed NSAIDs, controlling blood pressure and glucose. Very low-protein diets and the "kidney detox" products sold online have no place outside medical supervision — badly run, they cause malnutrition.
Do lifting weights or creatine supplements raise creatinine?
Both do, with no kidney injury: creatine converts spontaneously into creatinine, and more muscle produces more of it.6 Tell your doctor you take it — cystatin C settles the question when there is doubt.
Do I need to fast for a creatinine test?
Not mandatory, but a cooked-meat meal in the preceding hours measurably distorts the result — by roughly 0.2 mg/dL in the reference study.4 If your value is borderline, redo it fasting.
My creatinine is high but my BUN is normal — what does that mean?
Common, and reassuring about an acute problem. BUN rises mainly with dehydration, high protein intake or GI bleeding. An isolated high creatinine points instead toward muscle mass, a medication, or early kidney disease — the eGFR and the uACR decide.
My report has no eGFR. Is that normal?
