CRP High: What Your Number Means (mg/L vs mg/dL)
A high CRP says inflammation exists, not what it is. What 8, 40 or 120 mg/L supports, why mg/dL is ten times mg/L, and why no cutoff proves bacterial.
Your lab report flags a CRP above the reference range, and you have a number in your head: 0.8, 8, 24, 63, 140. The essential point is rarely printed first: CRP tells you inflammation exists. It does not tell you what it is. It names no cause, no organ, no severity — it is not a diagnosis. CRP high — but is it really abnormal?
Before anything else: check your unit
This is the single most useful line on this page. There is no standardized unit for CRP in the United States. StatPearls, the NIH reference, says so plainly: "Laboratory values for CRP vary, with no standardized reporting method currently in use. Results are typically presented in either mg/dL or mg/L."1 The two differ by a factor of ten:
mg/L ÷ 10 = mg/dL. So 0.5 mg/dL = 5 mg/L, 1.0 mg/dL = 10 mg/L, 8.4 mg/dL = 84 mg/L.
So a reported "CRP 10" is either an ordinary gray-zone result (10 mg/L) or a marked elevation (10 mg/dL = 100 mg/L). Same digits, two completely different conversations. Compare your number to an online range without matching the unit and you can be tenfold off — a mismatch that, far more often than any disease, turns a normal result into a frightening one. Read the unit printed next to your value, then the range printed next to that.
The gray zone is real, and it is wide
In healthy young adult blood donors, median CRP is 0.8 mg/L, the 90th percentile 3.0 mg/L, and the 99th percentile 10 mg/L.2 In other words, one perfectly healthy person in a hundred is already above 10 mg/L on any given day.
The band just above normal is crowded too. CRP between 3 and 10 mg/L (0.3–1.0 mg/dL) is "found in about one third of the American population," tied to dozens of banal things — body weight, diet, demographics — with no disease behind them.3 MedlinePlus is equally cautious: "It is unclear if a minor rise of 0.3 to 1 mg/dL in CRP levels is a cause for concern."4 A CRP of 6, 8 or 12 mg/L in someone who feels well is not a disease. It is the edge of the crowd.
U.S. sources also disagree about where normal ends, by a factor of three: below 0.3 mg/dL (3 mg/L) for StatPearls, 0.8–1.0 mg/dL or lower for MedlinePlus.15 The same 7 mg/L is therefore "high" at one lab and "normal" at another. Trust the interval printed beside your own result, not one from a website — this one included.
The pre-analytical trap here is timing, not the tube
Unlike potassium, where a bad draw is the leading cause of an abnormal result, CRP is a robust assay: no fasting is required1 (see fasting before a blood test), and a tight tourniquet or a hemolyzed sample does not manufacture a high CRP.
Its trap is the clock. After a stimulus, "serum concentrations rise above 5 mg/L by about 6 hours and peak around 48 hours."2 Two consequences explain most confusing results:
- a CRP drawn too early — the first evening of a fever — can be normal while the infection is entirely real;
- a CRP still high when you already feel better is the rule, not a warning: you are on the falling half of a 48-hour curve.

The "viral or bacterial" cutoff does not hold
The most repeated claim about CRP fails on the numbers. The reference systematic review of febrile children pooled five studies (1,379 children) and found a positive likelihood ratio of 3.15 (95% CI 2.67–3.71) and a negative likelihood ratio of 0.33 (0.22–0.49) for serious infection.6 Useful. Nowhere near decisive.
More telling, the same authors concluded that clinicians "should apply different cut-off values depending on whether they are trying to rule in or rule out serious infection": about 80 mg/L (8.0 mg/dL) to rule in, a much lower 20 mg/L (2.0 mg/dL) to rule out.6 One number cannot do both jobs — which is exactly what the myth of "the" CRP threshold assumes. A 2024 systematic review of reviews found the same in adults: biomarkers are only "slightly better" than symptoms and signs.7 And the 2022 Cochrane review (12 trials, 10,218 participants) tested point-of-care CRP as a way to cut unnecessary antibiotics, not as a test that names a germ.8
U.S. guidelines behave accordingly: ATS/IDSA recommends starting empiric antibiotics in radiographically confirmed pneumonia "regardless of initial serum procalcitonin level,"9 and the American College of Physicians and the CDC advise clinicians "should not perform testing or initiate antibiotic therapy in patients with bronchitis unless pneumonia is suspected."10
No CRP value proves "it's viral" or "it's bacterial." There is only a gradient of probability, which the clinical exam turns into a decision. Procalcitonin is more specific for bacterial infection — still not a verdict.
Standard CRP and hs-CRP: same protein, two different questions
This confusion sends people into the wrong panic. They are not two proteins, but one molecule measured by two assays for two purposes.
- Standard CRP asks: is there inflammation right now? Read in tens and hundreds of mg/L.
- hs-CRP asks: what is my background inflammation, for cardiovascular risk? Read in the low single digits, in the bands the American Heart Association and the CDC set in their joint statement:114

Those bands are mg/L — the one place the unit is settled. Hence a rule that surprises people: an hs-CRP above 10 mg/L is not read as enormous heart risk at all. At that level you are measuring an acute inflammatory event, not arterial background, so the move is to repeat at least two weeks later, back at baseline.11 If you searched "crp high heart risk" after a bad sore throat, that 40 mg/L is describing your throat, not your arteries.
The causes, from most common to rarest
Ordered by real-world frequency, not by severity:
- Ordinary respiratory and ENT infections. By a wide margin the leading reason — and most resolve on their own, which is why guidelines discourage reflex antibiotics.810
- A localized infection: urinary, skin, dental, gastrointestinal. CRP is often the first hint a focus exists, before anyone finds it.
- Injury, recent surgery, or hard exertion. Tissue damage is as strong a stimulus as any microbe: after a heart attack, CRP peaks around 48 hours in proportion to the damage.2 A high CRP three days after an operation is expected.
- A flare of a known inflammatory disease — rheumatoid arthritis, inflammatory bowel disease, other rheumatologic conditions.
- A raised baseline with no acute illness at all (below) — common, but only in small numbers.
- The rare causes: deep-seated infection, vasculitis, blood disorders, advanced cancer. Looked for when the picture does not add up; never deduced from the number alone.
What raises your baseline: real, but much smaller than people assume
These get listed in bulk. The measured size of the effect is the useful part:
- Age. Across nearly 5,700 adults, median CRP "approximately doubled with age, from approximately 1 mg/L in the youngest decade to approximately 2 mg/L in the oldest," slightly higher in women.12 Real — and tiny.
- Excess weight and obesity. One of the best-established drivers of a durable minor elevation.3 Again: the low band, not 60 mg/L.
- Smoking. A consistent association, but one that seems to run mainly through local, non-arterial inflammation — periodontal disease — rather than a direct effect of smoke.2
- Pregnancy. In 146 healthy women before 18 weeks, median CRP was 3.2 mg/L (IQR 0.3–12.1) and more than half were above 3.0 mg/L, the non-pregnant reference; the authors urge caution, to avoid unnecessary interventions.13
- Estrogen — where the route matters more than the drug. In a randomized, placebo-controlled crossover trial, eight weeks of oral conjugated estrogens produced a more than twofold increase in CRP, while transdermal estradiol had no effect in the same women — and neither route changed IL-1β, IL-6 or TNF-α.14 That last detail is the whole story: with no movement in the cytokines that drive CRP synthesis, this is not inflammation but a first-pass hepatic effect of swallowing the hormone. "Estrogen raises CRP" is too vague — oral estrogen does, which is why MedlinePlus also lists birth control pills among causes of a positive CRP.4
None of these produces an 80. They shift your baseline by a few mg/L, nothing more.
What should make you seek care without waiting
The number does not decide this; your symptoms do. Whatever your CRP, be seen promptly for:
- a fever above 101.3 °F (38.5 °C) for more than 48 hours, or with hard shaking chills;
- unusual shortness of breath, chest pain, or feeling faint;
- severe abdominal pain, or pain that localizes and worsens;
- skin that is red, hot, and spreading;
- confusion or abnormal drowsiness, especially in an older adult;
- fever while immunocompromised, on chemotherapy, without a spleen, or with a prosthetic joint or catheter.
A moderately high CRP with none of these, days into a head cold, is not an emergency.
What your doctor will do next
They do not treat the number. They look for the source.
- A focused physical exam — throat, chest, abdomen, skin, urine, teeth. That is what gives the CRP meaning.
- Targeted tests along the lead: a CBC with white blood cells and neutrophils, urinalysis, chest X-ray, liver function tests.
- A repeat CRP, usually more informative than the first.
- For prolonged, unexplained inflammation: ESR, ferritin, fibrinogen, serum protein electrophoresis — the wider picture in our inflammatory markers guide.
What this result does NOT mean
- It is not a cancer test. CRP is not a tumor marker and has no screening value.
- It is not proof of infection. A sprain, an operation, a joint flare all raise CRP with no microbe involved.
- It is not a severity score. A pneumonia with an excellent prognosis often runs higher than a serious chronic illness.
- It does not localize anything. No CRP value points at an organ.
- A normal CRP does not clear you either. Some inflammatory diseases — systemic lupus is the classic — "fail to elicit major CRP production despite evident inflammation and tissue damage."2
Should you repeat it, and when?
Usually yes: the trend, not the single value, carries the information. The plasma half-life of CRP "is about 19 hours and is constant under all conditions of health and disease,"2 so your level depends only on how fast your liver is making it right now. Once the stimulus stops, CRP roughly halves every day: 120 mg/L on Monday and 60 on Wednesday is a different story from 120 then 130.
- acute episode: recheck at 48–72 hours, to confirm it is falling;
- isolated moderate elevation, no symptoms: recheck in 2 to 4 weeks, once any minor illness has cleared;
- hs-CRP above 10 mg/L: draw no cardiac conclusion, and repeat at least two weeks later, at baseline.11

A high CRP is never read alone. Its meaning comes from the unit, your symptoms, your CBC and white blood cell count, and above all the direction it is moving.
