High Ferritin Levels: Causes and When to Worry

High ferritin levels usually mean inflammation, alcohol or fatty liver — not hemochromatosis. Transferrin saturation, not ferritin, is the test that decides.

Your lab report flags a high ferritin level and you want to know whether it is serious. Here is the fact that changes how you read that number: ferritin is not only your iron gauge — it is also an acute-phase protein. It climbs whenever something is going on: an ordinary infection, a liver under strain, a few too many drinks, extra weight. Which is why, in the overwhelming majority of cases, high ferritin levels are not hemochromatosis. First: is your ferritin really abnormal?

Compare it to the range on YOUR report, not to a website

There is no single American ferritin cutoff. Cleveland Clinic publishes adult ranges of 15–205 ng/mL for females and 30–566 ng/mL for males, and says plainly that "labs may have different reference ranges," since cutoffs depend on the age brackets and assay each lab uses.1

That spread matters: the thresholds American guidelines use to define iron overload — above 300 ng/mL in men, 200 ng/mL in women — sit well below the top of some lab ranges.23 A man at 400 ng/mL may see no flag; a woman at 220 sees one. The range printed on your own report is what your result was judged against.

A unit note: U.S. labs report ferritin in ng/mL, international papers in µg/L, and 1 ng/mL = 1 µg/L exactly — nothing to convert. Serum iron and TIBC, by contrast, are in µg/dL here and µmol/L elsewhere; those do differ.

Just over the line is not a diagnosis

A ferritin of 320 ng/mL in a man whose lab caps at 300 is 7% above the line — a number to recheck, not a disease. What concerns a clinician is a high level, a rising trend, or a telling clinical picture. The reference point guidelines actually use is 1,000 ng/mL: below it, the ACG notes, C282Y patients are at low risk of advanced fibrosis.3

What can push ferritin up with nothing wrong

Ferritin is stable through the day and needs no fasting. But it rises for reasons that are not disease:

  • a recent infection, even a trivial one — a cold, strep throat, a UTI. As an acute-phase protein, ferritin stays up for weeks after you feel better;4
  • iron supplementation, especially intravenous;
  • any acute inflammatory episode — a flare, the weeks after surgery.5

Hence the rule: a ferritin drawn during or just after an infection cannot be interpreted. Repeat it when you are well.

The causes, from most common to rarest

The American College of Gastroenterology puts it bluntly: among people with an elevated ferritin who are not C282Y homozygotes, fatty liver and alcohol-related liver disease are more common than hereditary hemochromatosis.3 The real order:

1. Inflammation and infection

The most frequent explanation, and the most banal. Any inflammation lifts ferritin alongside CRP and the sedimentation rate: a recent infection, a chronic inflammatory disease, an autoimmune disorder, the aftermath of surgery.54 Iron has nothing to do with it — the body sequesters iron into storage and blood ferritin follows.

2. Alcohol

Regular drinking raises ferritin long before cirrhosis or dependence — often alongside a raised GGT. Alcohol-related liver disease is one of the two diagnoses the ACG names as more common than hemochromatosis here.3 Tell your clinician honestly; it is the fastest way to avoid needless testing.

3. Metabolic syndrome and fatty liver (dysmetabolic hyperferritinemia)

The most under-recognized cause and, in the United States, probably the leading one. It pairs a high ferritin with features of metabolic syndrome — waist, blood pressure, borderline A1c, high triglycerides — plus fatty liver, usually with a raised ALT.

The numbers show its weight: hyperferritinemia is found in about one-third of people with fatty liver disease or metabolic syndrome, and dysmetabolic iron overload syndrome (DIOS) affects roughly 15% of people with metabolic syndrome and half of those with fatty liver — the most frequent iron-overload condition there is.6 Its definition is specific: hyperferritinemia with a normal or only moderately raised transferrin saturation, metabolic abnormalities, and mild hepatic iron excess on MRI.7

Counterintuitively, blood removal is not the established treatment here. Deugnier and colleagues say phlebotomy "cannot currently be considered a valuable option" in DIOS, putting diet, exercise, and control of blood pressure, glucose and lipids first;7 a later review is less categorical.6 Experts disagree; lifestyle is the part they agree on.

4. Liver injury

Any destruction of liver cells releases ferritin: viral hepatitis (see hepatitis C), drug-induced injury, steatohepatitis, cirrhosis of any cause.35 Here the transaminases lead — ALT and AST — which puts the liver panel at the center of the work-up. Chronic kidney disease belongs in this tier too.

5. Hereditary hemochromatosis — last on this list, not first

Readers' number-one fear, and statistically one of the last hypotheses. HFE hemochromatosis is a disorder of intestinal iron absorption, and it responds very well to treatment when caught before organ damage.8

How common is it in the U.S.? NIDDK reports that about 1 in 15 people of Northern European ancestry carry one copy of the C282Y mutation, and about 1 in 225 carry two.8 The best American data come from the HEIRS study, which screened 99,711 primary-care participants: 299 were C282Y homozygous — 0.44% of non-Hispanic whites, versus 0.027% of Hispanic and 0.014% of Black participants.9 Ancestry changes the prior enormously.

And penetrance is incomplete — the point almost nobody is told. Two copies is not the same as the disease. In 31,192 people of Northern European descent followed for an average of 12 years, documented iron-overload-related disease (cirrhosis, fibrosis, liver cancer, raised transaminases, symptomatic hemochromatosis, the characteristic arthropathy) occurred in 28.4% of male C282Y homozygotes and 1.2% of female homozygotes.10 Most men who carry it, and nearly all women, will never develop an iron-related complication. A positive genetic test means monitoring, not a verdict.

That is also why the U.S. does not screen for it: the AASLD states "average risk population screening for HH is not recommended,"2 and the USPSTF has inactivated the topic.11

6. Rare causes

Non-HFE hemochromatosis (rarer but more penetrant), ferroportin disease, transfusional iron overload, hematologic disorders, and — in a severely ill patient with ferritin in the thousands — hemophagocytic lymphohistiocytosis (HLH).4 Each is sought after the five above are ruled out.

The test that actually decides: transferrin saturation

Remember this: ferritin alone cannot tell inflammation from iron overload. Transferrin saturation (TSAT) can.

The threshold with authority in the U.S. is 45%. The AASLD recommends that if either iron study is abnormal — TSAT ≥ 45% or ferritin above the upper limit of normal — HFE mutation analysis should be performed.2 The ACG goes further: saturation above 45% is preferred over hyperferritinemia as the trigger for genetic testing,3 since that cutoff catches 97.9–100% of C282Y homozygotes. Treatment in a confirmed homozygote requires both: TSAT ≥ 45% with ferritin > 300 ng/mL in men or > 200 ng/mL in women.2

TSAT is not a separate draw: it is serum iron (µg/dL) divided by TIBC (µg/dL), from the same iron panel as your ferritin — it may already be on your report. U.S. practice on timing is more relaxed than European: the AASLD notes a fasting sample is "advisable" but "not absolutely necessary," since fasting improved neither sensitivity nor specificity.2

And if saturation is normal? That is the common outcome, and good news: HFE hemochromatosis becomes very unlikely, and the work-up turns toward inflammation, alcohol, liver injury and metabolic syndrome.3 One caveat: a normal saturation does not rule out iron overload in general. Dysmetabolic iron overload is defined by a normal or only moderately raised saturation with real hepatic iron excess on MRI.7 A normal TSAT excludes the commonest and most dangerous genetic overload — not excess iron itself.

What should make you seek care promptly

Most high ferritins are handled at a scheduled appointment. A few should not wait:

  • prolonged fever, drenching night sweats, or unexplained weight loss;
  • yellowing of the eyes or skin, very dark urine, a swelling abdomen, swelling legs;
  • pain in the finger joints, especially at the base of the index and middle fingers — a painful handshake — the arthropathy characteristic of iron overload;10
  • a gray or bronzed complexion without sun exposure, with unusual fatigue or newly diagnosed diabetes;8
  • a ferritin above 1,000 ng/mL in any context: the liver is evaluated without delay.32

Otherwise, an appointment in the coming weeks is the right pace.

What your doctor will do next

The path is standardized, usually one or two visits.34

  1. A history: alcohol, medications and supplements (including iron), waist and blood pressure, family history, ancestry.
  2. A simple panel, often on one draw: transferrin saturation, CRP, ALT / AST / GGT, A1c, triglycerides, and a CBC.
  3. If saturation is confirmed above 45%: HFE genetic testing for C282Y.23 If homozygous, treatment is therapeutic phlebotomy — NIDDK describes a pint once or twice weekly for months, then maintenance 2–3 times a year8 — targeting a ferritin of 50–100 ng/mL.2
  4. If saturation is normal: the cause is treated and ferritin rechecked. Genetic testing adds nothing.
  5. If ferritin exceeds 1,000 ng/mL, or transaminases are raised in a confirmed homozygote: liver staging — historically by biopsy, increasingly by MRI, which the ACG notes has removed the need for biopsy in most patients.23

What high ferritin does NOT mean

It is not a cancer test. Ferritin is not a tumor marker: no guideline recommends measuring it to look for cancer, and no threshold "signals" a tumor.

Be honest about the number that circulates. In a study of 2,044 inpatients with a ferritin ≥ 500 ng/mL at a tertiary center in Seoul, solid organ malignancies accounted for 37.1% of diagnoses, ahead of infections (23.4%) and blood disorders (14.9%).12 That figure is real — but it describes hospitalized patients in whom ferritin was drawn precisely because they were unwell. Transplanting it onto someone who feels fine and finds a ferritin of 380 on a routine panel is a base-rate error: how often a disease appears in a hospital is not its probability in you.

U.S. screening data make the same point in reverse. In HEIRS, among 364 participants with a previously undiagnosed ferritin above 1,000 µg/L, only 29 were C282Y homozygotes — about 8%.9 Even at the extreme end, the genetic disease people fear explains a small minority.

It is not "too much iron in the blood," either. Ferritin measures stores, not circulating iron, and can be high with entirely normal stores — exactly what inflammation does.4

And it is not hemochromatosis until transferrin saturation has spoken. Even genetically confirmed, it does not equal disease: penetrance is incomplete.10 Nor is it a reason to change a medication or start a supplement on your own.

Should you retest, and when?

Yes, usually — a single ferritin value says very little on its own.

  • After an infection or inflammatory episode: recheck 4 to 8 weeks after full recovery, with a CRP. If both have normalized, the matter is closed.
  • If alcohol is the likely driver: a recheck after several weeks of genuine reduction is both test and treatment.
  • In a metabolic context: recheck at 3 to 6 months, tracking weight, glucose and transaminases.
  • If saturation was high: it should be repeated and confirmed before any genetic testing.2

A ferritin that falls on its own was an inflammatory ferritin. One that stays high with a normal saturation points at the liver and metabolism; one that stays high with a high saturation leads to genetics. Three results, three paths.

Frequently asked questions

What ferritin level is dangerously high?
There is no magic number; a value just above your lab's ceiling means very little. Above 1,000 ng/mL the liver is evaluated systematically — that is the level tied to a real risk of advanced fibrosis.32 In between, transferrin saturation and your context decide, not the ferritin figure.
Do high ferritin levels always mean hemochromatosis?
No, and rarely. C282Y homozygosity affects about 0.44% of non-Hispanic white Americans and far fewer people of other ancestries,9 while a high ferritin is common. The ACG states that fatty liver and alcohol-related liver disease are more common than hemochromatosis among people with a high ferritin who are not C282Y homozygotes.3
Can high ferritin mean cancer?
Ferritin is not a tumor marker and is not used to screen for cancer. The studies where malignancy dominates involve hospitalized patients with very high ferritin,12 not routine outpatient panels.
Should I have blood removed, or donate blood, to bring it down?
Not automatically. Therapeutic phlebotomy is the treatment for confirmed HFE hemochromatosis, targeting 50–100 ng/mL.28 In dysmetabolic hyperferritinemia its benefit is unsettled — one expert review calls it not a valuable option, another includes it — though both agree on lifestyle change.76 Decide with your clinician; blood donation has its own rules.
Do I need to stop eating red meat?
No. NIDDK is explicit even for people who actually have hemochromatosis: "Most people with hemochromatosis do not need to change their diet to eat less iron," because diet has a small effect next to phlebotomy. What it does advise avoiding: iron supplements, vitamin C supplements (which increase iron absorption), raw shellfish, and — with cirrhosis — alcohol entirely.8
My ferritin is high and I'm exhausted. Are they connected?
Usually not directly: fatigue is far more typical of low ferritin — iron deficiency. Fatigue with a high ferritin sends your clinician after the cause of the elevation itself: inflammation, the liver, alcohol, or real iron overload.
