Low Ferritin Levels: Causes, Symptoms & What to Do
Low ferritin means depleted iron stores — the earliest, most specific sign of iron deficiency, and it shows up long before your hemoglobin does.
Your lab report shows a ferritin below your lab's lower limit — and your hemoglobin may be perfectly normal. That combination is the most common, and the most misread. Low ferritin does not mean you are anemic. It means your iron stores are empty or nearly empty, and it is the single most specific result laboratory medicine has for iron deficiency.12 This page covers why low ferritin levels are defined differently by different sources, the causes in order of real frequency, who genuinely needs an endoscopic evaluation, which symptoms the evidence supports (and which famous one it does not), and when to retest. First: is it really abnormal?
A ferritin of 4 ng/mL and one of 27 ng/mL both sit "below normal", and do not tell the same story. Compare yourself to the range on YOUR report: reference intervals depend on the lab's assay, on sex, and on age. One U.S. lab prints roughly 15–205 ng/mL for adult women and 30–566 ng/mL for men; the lab down the street may print 11–307 or 24–336. A number flagged "low" at one lab can be "normal" at another — which is why results from two labs should not be compared directly.
A note on units. U.S. reports give ferritin in ng/mL. The international unit µg/L is numerically identical — 1 ng/mL = 1 µg/L — so there is never a conversion to do. Serum iron, drawn on the same panel, is reported in µg/dL, and hemoglobin in g/dL.
Ferritin is otherwise a robust measurement: no fasting needed, little drift, none of the tourniquet artifacts that plague potassium. Two things do distort it: a recent iron supplement, and a recent or ongoing inflammatory episode — see below.
The threshold is genuinely disputed: 15, 30, 45 or 70 ng/mL
There is no single agreed cutoff for iron deficiency, and saying so is more honest than inventing one.

The Cochrane review run with the WHO measured the 30 µg/L cutoff in adults presenting for medical care: specificity 98% — a ferritin under 30 is very rarely wrong — but sensitivity 79%, so a ferritin above 30 does not rule deficiency out (low-certainty evidence). The same review notes that cutoffs proposed across published studies ranged from 12 to 200 µg/L: the clearest demonstration that there is no consensus.1
This is not academic: NHANES 2017–2020 data put absolute iron deficiency at 14% of U.S. adults, and 11% among those with no anemia, heart failure, kidney disease, or pregnancy.6
The reverse trap: a "normal" ferritin rules nothing out
Ferritin is also an acute-phase protein. Infection, an inflammatory flare, obesity, liver disease, chronic kidney disease, heart failure, and cancer all push it up, independently of your iron stores — so a ferritin of 85 ng/mL alongside a raised CRP can hide a real iron deficiency.12
When that happens, clinicians stop reading ferritin alone and look at transferrin saturation — below 20% in deficiency — and sometimes at the soluble transferrin receptor, which inflammation does not raise.23 Low ferritin with a normal CRP is a straightforward reading; with a high CRP, the number has to be interpreted carefully, in both directions.
A normal hemoglobin does not mean "all fine"
This is the misconception to undo. Iron deficiency develops in order: the stores empty first (ferritin falls), red-cell production degrades next, and anemia appears last.27 A low ferritin with a normal hemoglobin and a normal MCV is therefore an early stage — iron deficiency without anemia — not a false positive. No source puts a reliable timescale on that sequence, and anyone who gives you one in months is inventing it.
It is also the majority situation: in high-income countries, roughly 38% of non-pregnant women of reproductive age have iron deficiency without anemia, versus 13% with iron-deficiency anemia.2
Can that early stage cause symptoms? The evidence differs sharply by symptom:
- Fatigue — the evidence points both ways, and both halves matter. A meta-analysis of six randomized trials found a significant effect of iron on fatigue in people iron-deficient without anemia; across six cross-sectional studies, the same authors found the association between non-anemic iron deficiency and fatigue in the general population not significant.8 Iron helps the genuinely deficient; it does not explain fatigue in general. - Restless legs syndrome — moderate-certainty evidence. A Cochrane review of ten trials found iron probably improves restless legs severity versus placebo.9 RLS affects an estimated 32–40% of people with iron deficiency.2
- Shortness of breath — plausible, poorly demonstrated without anemia. Dyspnea and exercise intolerance appear on every symptom list for non-anemic iron deficiency,2 but frank breathlessness tracks hemoglobin, and the evidence separating the two is thin.
- Hair loss — NOT an established symptom. This is the claim most often attached to low ferritin, and the least supported. A review in the Journal of the American Academy of Dermatology found insufficient evidence to recommend screening for iron deficiency in patients with hair loss, and insufficient evidence to recommend iron supplementation for hair loss in the absence of anemia.10 A meta-analysis of 36 studies did find that women with non-scarring hair loss have lower ferritin on average11 — the association exists in observational data; the proof that correcting iron regrows hair does not. Contradictory and insufficient is the honest summary.
- Also recognized: brittle nails, cold intolerance, and pica — cravings for ice or clay.212
The causes, from most common to rarest
Your body loses iron only through bleeding and shed cells, and gains it only through the gut. Every low ferritin comes down to three mechanics: losing it, not absorbing it, or needing more than you take in.
- Heavy menstrual bleeding — by far the leading cause in people who menstruate. Long periods, clots, or protection that needs changing hourly drain the stores year after year without ever producing frank anemia.212
- Pregnancy — requirements rise steeply in the second and third trimesters; in high-income countries, iron deficiency reaches up to 84% of pregnant women by the third trimester.2 The USPSTF concluded in 2024 that evidence is insufficient to judge routine screening and supplementation in asymptomatic pregnant people — an I statement: "not enough evidence," not "don't."13
- Inadequate intake — diets low in bioavailable iron, poorly compensated vegetarian or vegan patterns, restrictive dieting, adolescent growth. The NIH Office of Dietary Supplements sets the RDA at 8 mg/day for adult men and 18 mg/day for women aged 19–50, and notes that people who eat no meat, poultry, or seafood need nearly twice that, since non-heme iron is absorbed less well.14
- Malabsorption — including celiac disease, which is regularly missed. Iron is absorbed in the duodenum, so anything that damages it blocks absorption. A meta-analysis of 18 studies found biopsy-confirmed celiac disease in about 1 in 31 patients with iron-deficiency anemia (3.2%; 5.5% in the higher-quality studies) — enough to justify testing.15 Both the ACG and the AGA put serologic testing (tissue transglutaminase IgA) first.164 Also here: H. pylori gastritis, inflammatory bowel disease, bariatric surgery, long-term proton pump inhibitors.2
- Repeated blood donation — each whole-blood donation removes a substantial share of your stores.
- Gastrointestinal bleeding — ulcers, esophagitis, angiodysplasia, polyps, more rarely colorectal cancer. It can be completely invisible: no visible blood, no pain, no change in bowel habit. This is the cause that drives the workup below.
- Rare causes — intravascular hemolysis, urinary blood loss, genetic disorders of iron absorption.
What should make you see a doctor promptly
An isolated low ferritin is not an emergency. These situations warrant a prompt appointment:
- You are an adult man, or a postmenopausal woman. No menstrual loss explains your deficiency, so the cause has to be found — and it is frequently gastrointestinal.47
- Blood in your stool, black tarry stools, or vomiting blood.
- Unexplained weight loss, a lasting change in bowel habit, or ongoing abdominal pain.
- Breathlessness on minimal exertion, chest pain, or fainting — these suggest anemia is already established.
- Marked pallor with palpitations.
What your doctor will do next
The path always has two halves: correct the deficiency, and find out why it happened.
1. Complete the workup. Usually a CBC (any anemia? are the red cells small?), a CRP (is inflammation distorting the ferritin?), and the rest of the iron panel: serum iron in µg/dL, transferrin or TIBC, transferrin saturation. On the CBC, a rising RDW is often the earliest fingerprint, then a falling MCV and MCHC. With anemia, vitamin B12 and folate are often added — see anemia blood tests.
2. Look for the cause. Questions about periods, diet, NSAIDs and anticoagulants, blood donation — plus celiac serology, which the AGA and ACG both favor over going straight to biopsy.416
3. Evaluate the GI tract — in whom, exactly. Here clarity beats reassurance. The AGA 2020 guideline on the gastrointestinal evaluation of iron-deficiency anemia recommends bidirectional endoscopy — upper endoscopy plus colonoscopy — in asymptomatic postmenopausal women and adult men. The reason is concrete: colonic malignancy is found in about 9% of these patients and upper GI malignancy in about 2%, more than ten times the yield of routine colorectal screening.45
For premenopausal women, the same guideline only suggests it — a conditional recommendation on very low-certainty evidence, since malignancy turns up in about 1%. In a menstruating woman with clearly heavy periods and no alarm features, GI workup is therefore not automatic: a trial of iron replacement first is a recognized option, with investigation if the deficiency recurs or resists.45 These recommendations were written for iron-deficiency anemia; with low ferritin and no anemia, the decision is more individual.
4. Treat. Oral iron first in most cases; intravenous iron when oral iron is not tolerated or absorbed, when losses are ongoing, or in the second and third trimesters of pregnancy.2 Dose and duration are your physician's call — iron taken without a proven deficiency achieves nothing and risks overload.
What this result does NOT mean
- It is not anemia. The two are distinct, and most low ferritins come with a normal hemoglobin.
- It is not cancer. Iron deficiency justifies looking for a GI source in certain people — not the same as having one. In a menstruating woman, the cause is almost always menstrual.
- It is not proof your fatigue comes from this. Iron deficiency is a frequent cause of fatigue, not the only one, and correcting it does not always resolve the symptom.8
- It does not explain your hair loss. The evidence for that link is insufficient and contradictory.1011
- It is not a reason to self-supplement. Over-the-counter iron distorts the next test and can mask the underlying cause.
- And conversely: a normal ferritin is not an all-clear if your CRP is high.
Should you retest, and when?
Before treatment, an immediate recheck adds little — ferritin does not swing from draw to draw. One exception: if the sample was taken during or just after an infection, repeating it 4 to 6 weeks later gives a truer picture.
On treatment, recovery comes in stages — worth knowing so a still-low ferritin at the first recheck does not alarm you.
- reticulocytes rise within roughly a week;
- hemoglobin recovers next, with a clear rise expected after about 4 weeks of well-taken treatment;
- ferritin — the stores themselves — recovers much more slowly. Treatment is therefore continued well beyond hemoglobin normalization, with ferritin rechecked around 3 months and again after stopping.
If ferritin does not rise despite treatment taken correctly, two explanations: the iron is not being absorbed (malabsorption, celiac disease), or the losses continue (active bleeding).
